When children, youth and families face challenges such as homelessness, hunger, mental health, or expulsion—navigating multiple providers, and requesting them to coordinate, can be overwhelming.

The Collaborative Management Program provides case management multi-agency services for children and youth 0-21—and their families—when facing challenges. Families remain active partners throughout the CMP process, ensuring services are responsive to family priorities.
